Now select the Aadhar radio button from the "Enter your Personal Details" portion. Now enter aadhar number, full name & pin code (your address pin code) & select the "Masked Aadhar" option. This is to protect your privacy and helps you in hiding your aadhar full digit. Enter security code & click "Request OTP" to receive a one-time password on your registered mobile phone number. After that. You have to click "I agree" to provide personal details for the authentication process. Click the confirm button, this way you will receive OTP on your mobile number as registered in the UIDAI database. Now enter the OTP received and click "download aadhar card" to download the aadhar card.

On the next page, choose VID from the "I Have" section. After that, enter VID, Full Name, Pin Code & Security Code. Now click request OTP button to receive OTP on your registered mobile number. Once you get the OTP, enter it & download the aadhar card for print.

There is no hard & fast rule to have aadhar print on the lamination sheet. Just have it on plain white paper to fulfill the requirement of Government.
